Stage 1: freeze supplied questions, answers, and rules
Start from approved source content, not a draft pasted into several messages. Retain the exact learner-visible wording and distinguish it from the internal question title that administrators use to find the item. LearnDash requires points for quiz questions, so every scored question needs an approved points value. The implementer can identify a missing or contradictory rule, but should not quietly become the subject-matter expert who decides which answer is correct.
Stage 2: place each quiz in the correct learner path
Record whether the quiz is final to the course, attached to a lesson or topic, or used outside a course. LearnDash supports course and lesson associations, but Shared Course Steps changes where those associations are managed. Prerequisite quizzes, release schedules, registered-user access, and linear course progression can also change whether a learner can reach or pass the step. Placement should therefore be named before questions are entered.
Stage 3: choose supported question types without changing the assessment
LearnDash documents single choice, multiple choice, free choice, sorting, matrix sorting, fill in the blank, assessment, and essay question types. Those labels are not interchangeable. Multiple-choice items need every correct option identified. Free-choice and fill-in-the-blank items need approved answer patterns. Sorting and matrix items need a complete ordered or paired structure. Assessment items are not ordinary correct-answer questions. Essay questions introduce grading and progression ownership and are outside this simple package unless separately scoped.
Stage 4: build questions with searchable internal names
Use Quiz Builder to add, edit, reorder, or remove questions while preserving a stable internal naming convention. Enter learner-visible text separately, add answer options in the intended order, mark the approved correct answers, set required points, and add only supplied feedback or hints. Removing a question from one quiz does not necessarily delete the underlying question object, so cleanup decisions should be explicit rather than assumed.
Stage 5: configure passing, access, retakes, and completion
Quiz access and progression settings are per quiz. Record the passing score between 0 and 100; LearnDash notes that a passing score of 0 allows continuation after submission regardless of score. Decide whether all questions are required, whether retakes are unlimited or restricted, whether a time limit applies, and whether quiz saving is enabled. Current LearnDash guidance warns that very short autosave intervals can create incomplete writes under heavier traffic and recommends 30-60 seconds when saving is used.
Stage 6: make display and result behavior intentional
Choose one-question-at-a-time or all-questions-at-once presentation, order and randomization behavior, visible points or numbering, title output, answer review, correct or incorrect marks, custom feedback, result messages, and score details. LearnDash warns that enabling the quiz title can duplicate a title already printed by the WordPress theme. When retakes are allowed, revealing correct answers immediately can also undermine the intended assessment. These are owner decisions, not defaults to toggle blindly.
Stage 7: test as a representative learner
Administrator preview alone is insufficient. Use a clearly labelled, approved QA user with the correct enrollment and role state. Verify the learner can reach the quiz from its intended course location, see the expected title and questions, submit required answers, pass and fail according to the approved threshold, use or be denied a retake correctly, receive the intended result display, and continue or stop as designed. Repeat the main path on a narrow mobile viewport and record unexpected theme or plugin behavior instead of hiding it.
If quiz statistics are needed, enable and verify the setting before the relevant attempts; LearnDash states that statistics are not retroactive for attempts completed while collection was off. Email delivery, certificates, leaderboards, essay grading, and third-party reporting each add their own data, privacy, operational, or deliverability decisions and remain separate unless explicitly included.
Stage 8: hand off an inventory, not a vague completion message
The handoff should list every quiz and URL, placement, question count, question types, points, passing score, retake rule, required-question state, time limit, saving state, display mode, result behavior, statistics state, QA identity, pass/fail evidence, known exception, separate owner, and review date. It should also confirm whether temporary access was removed. This makes later curriculum edits, learner support, and release QA possible without reverse-engineering the builder.
